Five W's of Hospice

  • Who is Eligible?

    Any patient with a terminal illness resulting in a life expectancy of six months or less, as determined by the patient’s physician, is eligible for hospice care. The patient, in partnership with his or her family, the referring physician and the hospice team, makes the decision for hospice care, after a thorough review of all the options for care.  

  • When is it Time?

    The sooner hospice care begins, the sooner the patient and family are able to benefit from the full range of services offered by the Helping Hands Hospice. The time to choose hospice care is when the goal of treatment begins to shift from curing the illness to providing comfort. By contacting Helping Hands Hospice early in the diagnosis, the patient and family reserve time to understand their options and choose the path that is best for them.  

  • Where is Hospice Care Provided?

    Most hospice care takes place where the patient lives and where the patient is most comfortable. That might be their own home, a family member’s home, or an assisted living facility. 

  • What Does Care Include?

    Helping Hands Hospice provides care that focuses on the relief of suffering in all its forms: physical, emotional, spiritual, and social. Our team of professionals works with the patient and family to create a plan of care unique to their situation. That plan might include: Home visits by nurses, physicians, home health aides, and social workers. Medications, supplies and durable medical equipment related to the terminal illness. Caregiver support and education. Bereavement care.

1. Call

 If you have an emergent need, please call us at (575) 461-0099 or our on call number at (575) 403-8680

2. Complete a Referral

Anyone can make a referral. Once the referral has been completed, you can expect to hear back from us within one business day. In order to be admitted, the patient must have a physician’s order. We are more than happy to reach out to the patient’s health care provider in order to get the necessary information to initiate services. 

3. Have an Initial Visit

At this visit, the patient and caregivers will work with our admissions team to complete an assessment, order medications and begin the patient’s personalized plan of care.
